SBTM-REG-001 – Registration of a new group, a new SOBATAM Pool for families group or a new SOBATAM Pool member
SBTM-REG-002 – To update information of members of an existing group or to add members to an existing group

SBTM-DEATH-001 (pdf) – To notify a death of a member.
SBTM-RRW-001 (pdf) – To request a Residency Requirement Waiver.
